Congratulations on your decision to join the Dreams Affiliate Program!

DO NOT COMPLETE THIS ONLINE ENROLLMENT IF YOU HAVE PREVIOUSLY BEEN APPOINTED WITH Dreams Affiliate Program.

The process to join the Dreams Affiliate Program will take you about 5 minutes.  To complete the application you will need the following information:

1. The recruiter code number of the Dreams Affiliate Solutions associate that invited you to join the company.  This number is in the following format:  aa1234 (the letters "aa" followed by four numbers). If you do not know this number, you can signup under a default code number.  It can be adjusted later.

2. Your Social Security Number (This is required to complete Federal Form W-9)
